PROVIDER REGISTRY

WHAT IS THE IHSS PUBLIC AUTHORITY PROVIDER REGISTRY?

The Provider Registry recruits and maintains a database of providers who are able to provide in home care to In-Home Supportive Services (IHSS) Recipients in our community. The Provider Registry does not hire or recommend IHSS providers, but creates lists of eligible and qualified providers who closely match the needs of the IHSS Recipient based on the information given to the Registry staff by both the IHSS Provider and the IHSS Recipient. The IHSS Recipient is responsible for interviewing, hiring, and terminating the IHSS Provider.

IN-HOME SUPPORTIVE SERVICES RECIPIENTS

IHSS recipients or their authorized representative can call the IHSS Public Authority Provider Registry directly at (866) 313-1353 to request a list of potential providers. A list of potential IHSS providers that best match the needs of the IHSS recipient will be mailed to the IHSS recipient or authorized representative.

The IHSS recipient is the employer – not the provider registry. IHSS recipients are responsible for interviewing, hiring, and supervising the IHSS provider they chose.

To designate a new IHSS provider, the IHSS recipient must complete Form SOC 426A - Recipient Designation of Provider (EnglishSpanish) and return it to the Public Authority. The recipient can call the IHSS Public Authority at (866) 313-1353 and ask to receive a Form SOC 426A by mail.
