CPSP Application Process

Please contact Louise Davis, Perinatal Services Coordinator, at (805) 681-5468 or louise.davis@sbcphd.org before completing the application to arrange an initial site visit.  No application will be accepted without a site visit.

You may download the CPSP application at http://www.cdph.ca.gov/programs/CPSP/Pages/ApplicationforCertificationasaCPSPProvider.aspx.  Instructions for filling out the application as well as the Provider Handbook are available at this website. 

The completed application will be reviewed by the PSC, a process which may take up to 30 days.  The PSC will then submit the completed application to State CPSP for final approval, which may take up to 60 days from the date the State receives it.
