EMT Certification:  EMT Certification Packet

 
EMT Initial Certification - $164.00 

Please bring a copy (front and back) of the following:

  • Valid Health Care Provider CPR Card (American Heart Association, Red Cross, or equivalent. Must be State EMS Authority approved). Online programs are NOT accepted. (copy)
  • Valid Driver License or another government issued photo ID. (copy)
  • EMT Course Completion Record (issued within the past two years). (copy)
  • Valid National Registry certification card AND certificate for all initial EMT applicants and EMTs that have a certification lapse of more than 12 months. (copy)
  • Completed Live Scan Application (Live Scan must be completed before application is submitted). (copy)            NOTE: When a Live Scan is completed the electronic results sent to the EMS Agency is only held for 2 months.

EMT Certification Renewal - $94.00

  • (24) hours of continuing education hours or (24) hour refresher course certificate (issued within the past two years). (copy)

EMT regulations changed July 1, 2010. The regulations affect new and recertifying EMT's.  All applicants for initial EMT certification must complete both a California DOJ and an FBI Live Scan.  Recertifying EMT's only need to complete the DOJ and FBI Live Scans if they are changing to a new certifying agency or have lapsed more than 12 months.

 

EMT Skills Competency Verification Form (For Re-Certification Only) EMT Skills verification form

  • This form must be completed by an EMT Instructor or an approved EMT Continuing Education Provider, with a valid provider number.  (A list of approved skills provider instructors for Santa Barbara County is available on our website).

Lapsed EMT certification:

  1. If your card has expired within the last 6 months, you will need course completion records for 24 hours of EMT approved continuing education hours.
  2. For lapses of more than 6 months but less than 12 months, you will need course completion records for 36 hours (12 additional hours) of EMT approved continuing education hours.
  3. For lapses of more than 12 months but less than 24 months you will need course completion records for 48 hours (24 additional hours) of EMT approved continuing education hours, plus completion of the National Registry Exam plus new Live scan.
  4. For lapses of more than two years, you will need to repeat the initial EMT course (160 hours), plus completion of the National Registry Exam plus new Live scan

      
All EMT continuing education must have an approved EMT continuing education provider number.
