SUPPLEMENTAL INSURANCE PROGRAMS 

 

  • Employee Assistance Program & CareCounsel Healthcare Assistance Services – You can continue your EAP coverage under your COBRA coverage described above.  You will be billed for this service by BCC.  If you choose to enroll you will be able to contact Empathia EAP at 1-800-367-7474, Company code COSB.
  • County-Provided Life Insurance Plan – all employees are covered by a County-provided term life insurance plan.  Most employees have a $20,000 policy with assistant department heads and department heads at $30,000 and $50,000 coverage respectively.  You may continue this policy as a whole life policy through VOYA ReliaStar Life Insurance Company.  If you are interested in converting this policy, please contact Employee Benefits at 1-805-568-2803 or 1-805-568-2818.  Please note that you have 31 days from the last day of the month in which you separated to convert this policy.
  • County-Provided Long Term Disability Coverage (LTD) – You may continue this plan after you separate through VOYA ReliaStar Life Insurance Company.  If you are interested in converting this policy, please contact Employee Benefits at 1-805-568-2803 or 1-805-568-2818.  Please note, this benefit is not offered to employees who terminate as Retirees or have been employed less than 1 year.  The insurance company must receive your written application and payment of the required premium within 31 days from the date of separation.

    • VOYA Optional Term Life Insurance Plan – If you currently have this term life insurance for you (and your spouse & children), you may continue it at the same premium and benefits through the portability feature.  This plan is provided through VOYA ReliaStar Life Insurance Company.  If you are interested in converting this policy, please contact Employee Benefits at 1-805-568-2803 or 1-805-568-2818.  If you have any questions, you may call VOYA Customer Service at 1-800-955-7736. Please note that you have 31 days from the last day of the month in which you separated to convert this policy.

    • VOYA Compass Critical Illness Insurance – If you currently have this insurance for you (and your spouse & children), you may continue it through the portability feature.  This plan is provided through VOYA ReliaStar Life Insurance Company.  If you are interested in converting this policy, please contact Employee Benefits at 1-805-568-2803 or 1-805-568-2818.  If you have any questions, you may call VOYA Customer Service at 1-800-955-7736.  The insurance company must receive your written application and payment of the required premium within 31 days from the date of separation.

    • VOYA Compass Accident Insurance – If you currently have this insurance for you (and your spouse & children), you may continue it at the same premium and benefits through the portability feature.  This plan is provided through VOYA ReliaStar Life Insurance Company.  If you are interested in converting this policy, please contact Employee Benefits at 1-805-568-2803 or 1-805-568-2818.  If you have any questions, you may call VOYA Customer Service at 1-800-955-7736.  The insurance company must receive your written application and payment of the required premium within 31 days from the date of separation.

    • VOYA Personal Accident Insurance – You may convert your VOYA policy to a private policy at different rates, after employment, and if you are under the age of 80.  For more information, please call 1-800-421-3005 (9am - 5pm EST) and ask to speak to Elisa Pietanza at extension 320 about converting your County of Santa Barbara PAI policy, to a private policy.  The insurance company must receive your written application and payment of the required premium within 31 days from the date of separation.  No evidence of insurability is required to obtain the conversion policy.  The maximum conversion amount is $500,000 and the least amount is $100,000.

Manhattan Life Insurance (previously HUMANA) – If you currently have this insurance for you (and your spouse and children), you can continue this benefit through the portability feature.  This plan is provided by Manhattan Life.
